Johnny Manziel is being investigated by police after an incident left his former girlfriend with a ruptured eardrum.
Colleen Crowley filed an affidavit against the Cleveland Browns quarterback last week, and details of it were released to the media on Monday.
Crowley won a protective order against the 23-year-old and Dallas police are investigating allegations of domestic violence – Manziel has not been charged or arrested.
The alleged incident took place in Manziel’s Dallas hotel room in the early hours of January 30, with Crowley claiming Manziel restrained her after throwing her on to his bed.
Later she alleges Manziel drove her to her home in Fort Worth, and on the way hit her so hard on the left ear that she suffered the injury and lost her hearing.
Manziel is also alleged to have threatened to kill Crowley and himself in the affidavit.
The protective order means Manziel is prohibited from contacting Crowley for two years and requires him to pay $12,500 (£8,625) in attorney’s fees for her.
