The 27-year-old, who was captain of the Viola, had long been linked with a move away from the Artemio Franchi, where he had played at since 2005 when he joined from Atalanta.
Liverpool were reportedly keen on signing him over a year ago but it is Milan who have now secured his services, much to delight of his family.
"To the delight of his father Marcello, a big fan of Gianni Rivera, and his entire family who like many people in Bergamo are Rossoneri fans, Riccardo Montolivo is a Milan player," a statement on the club's official website read.
"Montolivo has moved from Fiorentina where he had been captain since 2010 to join the most-titled club in the world."
The Italy international is the second signing that Milan have announced since finishing the season second behind Juventus in Serie A, after they completed the transfer of Bakaye Traore from French outfit Nancy.
"Bakaye Traore will be the first player from Mali to wear the Milan shirt," the club announced.
"Arriving from Nancy, the Malian international is a midfielder with physical strength, good running style and a remarkable shot."
Milan failed to retain the Scudetto this season after finishing four points behind the Bianconeri, and have immediately set out to reinforce their team ahead of the coming campaign.
